- Obtaining your provider’s Patient Registration Code and then visiting www.OrthoPacks.com/Registration/Patient to complete the required fields in the registration form. At the bottom of the form, click Register.
- Your provider can also complete the registration form on your behalf. Please Note: Your provider will need to create a temporary password for you. After your first log in, change your password under the General tab in My Account.
1. Your provider has the option to use your default shipping and billing information to place an order on your behalf. Your provider may also send the order to you to review and input your shipping and billing information. You can select to Accept or Decline the order.
If you accept the order, you will be taken to the checkout page to complete the order.
